Middle School (7th and 8th Grade) Attendance Form

When reporting a student's absence, please be specific about why your student will not attend school today. If reporting an ill student, Please include the symptoms in the "reason for absence" field on the form.

This attendance form is only valid if submitted using a parent or guardian email recognized by the Student Information System (SIS).
Are you submitting a full day absence, a late arrival, or early release?
Will the student be returning later in the day?
If reporting an ill child, include symptoms, especially if they have fever, nausea, vomiting, sore throat, and/or cough.  If your child has a diagnosis from a helthcare provider, please include that information.
